Monitor Affidavits


Monitor Information


Students who are taking a course CAN NOT be their own monitor.

A general definition of a monitor by most states is:

An impartial "disinterested third party" who verifies identity and processes an affidavit testifying the individual taking the examination received no outside assistance. A disinterested third party is someone with no family or financial relationship to the individual who is taking the examination.

Upon completion of the exam, the required monitor affidavit (see below table) must be faxed to Broker Educational Sales & Training, Inc. at (727) 372-7585. Failure to submit an affidavit will delay course credits being processed and could result in loss of credit for the examinee. All monitored exams must be closed book.

Alabama

The examination must be administered by an impartial third-party proctor, who must sign an affidavit attesting to the fact that the student received no outside assistance in the completion of the final examination.

Arizona

Self-study exams must be monitored by a CE provider director or a designee of the provider who is an Arizona licensee who supervises the student completing an exam.  The monitoring process must ensure that the student will complete the exam and that the specified conditions of administration are observed.  Exams must be kept sealed until the exam starts. The monitor and licensee must sign the Affidavit of Personal Responsibility.

Arkansas

A Disinterested Third Party must proctor all correspondence course examinations.  Proctors must provide an affidavit attesting under oath or affirmation that the correspondence or electronic course examination was proctored, that the examination was provided in a manner specified by the correspondence course provider, and that they are not part of or aware of any efforts to circumvent the requirements of the examination.

Connecticut

Self-study course may receive continuing education approval provided they include an examination on course material approved by the commissioner and administered by an impartial and disinterested person who shall not be in the direct line of supervision of any person taking the examination, nor have any financial interest in the success of any person taking the examination.

(1) Is related to, or is a business associate of, the licensee taking the examination; or
(2) Has a financial interest of a licensee taking the examination.

Indiana

When taking the self-study examination, the producer shall do all of the following:
(A) Sign an affidavit, supplied by the provider, that states the producer did not use outside help such as an open textbook or another individual, in taking the examination. 
(B) A second producer must sign the affidavit verifying that the second producer witnessed the first producers examination and no outside help was used.

A producer who takes the examination at a testing center that administers tests for professional designations may have a representative of the testing center sign the affidavit rather than a licensed producer.

Massachusetts

Self-study examinations must be proctored by an approved disinterested third-party.  The proctoring process must ensure that the examination will be completed by the student, on a closed-book basis without assistance, and the specified conditions of administration are observed.

Michigan

It is the providers responsibility to assure licensees complete exams without assistance.  Providers must use a disinterested third party to certify this action or schedule separate testing sessions at a specified location.  A disinterested third party is a person who has no financial interest in the success or failure of a licensee maintaining an insurance license.  The disinterested third party must also certify the exam was closed book and no other assistance was used to obtain a passing score on the exam.

Mississippi

Self-study final exams are required to be monitored by a disinterested third party (i.e. someone with no insurance business or family relationship to the student).

Missouri

Self-study exams must be proctored by a Disinterested Third Party Monitor.  Open book examinations will not be allowed.  The licensee will not be allowed access to books, notes, or any other reference material or information that would give them the answers to the examination questions.

Montana

A course examination monitor must be a disinterested third party.  The monitor cannot be a member of the licensees family by birth, marriage or any other recognizable family relationship.  The monitor may not have financial dependence on the licensee; this excludes employers, employees, business partners or any other person who would benefit from the licensee passing the examination from monitoring examinations.

Nebraska

An approved proctor is an impartial, disinterested third party or currently licensed agent with no family or financial relationship to the student. The proctor shall verify the agents identity and complete an affidavit supplied by the approved provider testifying that the agent received no outside assistance.  Membership in a professional association/organization does not constitute a financial relationship.
